The straightforward answer is that the Duggars are paying it. They have health insurance that covers standard check-ups, but does not cover maternity, so it isn't making a large impact on their payments. It is possible that, since Josie is a micro-preemie, they qualify for some disability papers, but the Duggars tend not to turn to public money.
