You do not have to be a supervisor to flag answers.
You do not flag questions, if there is something wrong with a question you click on 'report abuse' and give your reasons. It will then be taken care of.

It depends on what flag you are talking about. The US flag, for instance, has 50 stars. The flag of the state of Texas has one star. Please restate the question, as there is insufficient information in the question to properly answer it.

The supervisor will usually look at the question - and remove the flag if necessary.
The easiest way to complain is just flag the question/answer that needs work. There is a flag above the question, and that is what you click if the question, answer, or accompanying photo needs work. Or you can give the link to a supervisor. If a supervisor is responsible for the problem, then give the link to a community assistant. Or you can post the links you find offensive or in need of work to the vandalism discussion page.

You can use the flagging system to alert it to a supervisor. Click on "Flag" on the blue menu and select the appropriate flag and add a comment if necessary. The question now appears in that category as a "flagged" question (or answer, more likely). This should attract the attention of a supervisor who will examine the question and answer, and remove it, modify it, merge it, revert it- or, if it is fine as it is, unflag it.
We flag questions because instead of dealing with the question, if you are not sure how to improve the answer, you flag it so others can deal with it.
Yes this actually happens all of the time. Sometimes when a question is asked it is blatant nonsense or does not have enough clarification or information to be answered. You can flag it for review and a supervisor will see if it needs trashed/deleted.
Contributors cannot, but supervisors can. If you find a question that is inappropriate, unable to be answered, offensive, harmful, or illegal, then flag it or contact a supervisor.
