I got my attack up from killing flesh crawlers in the Stronghold for Security, but some people with the right combat level (65-80) could train on Green Dragons, because you can get some good drops and make decent money. Although if you go for the dragons make sure you have an anti-dragon shield. You can get 1 from Duke Horacio in Lumbridge if you did Dragon Slayer.

To wear Dharok's equipment, you need 70 defence; for the greataxe, you need 70 attack and 70 strength.
An Abyssal whip required level 70 attack to be equipped.
Your combat level is also dependant on your hitpoints, prayer, range, magic and summoning level. Assuming you had 70 attack, strength and defence, with 10 hitpoints (the lowest level you can have) and the rest of the skills mentioned above at level 1, you would have a combat level of 65. If however you had 70 hitpoints as well, you would be a level 80 combat.

You have to do quest to wield some weapons. Idk what your attack is but you should have it at level 60+ by Level 70. So in that case you would need to start getting Dragon Weapons and you have to complete the quest that Runescape shows you to get those weapons.

In Runescape there are monsters that attack at sight. Some attack you no-matter your level, and others attack you depending on your level at sight. Hell hounds attack you no-matter your level or by your level, but technically they will always attack at sight because "attack-at-sight" monsters attack people that are within two times their combat level. Hell Hounds have over 70 combat, so it is not possible to be safe from their attacks besides, safing behind certain areas.
Delrith has a combat level of 27 but has only 70 lifepoints.
