The best exercise for very obese people is swimming. The water supports their weight, so that they can move around without excessive difficulty. The very obese can float as easily or more easily than thinner people. Of course, not everybody has access to a swimming pool, so the best exercise may not be available. Alternatively, lie on your back and move your limbs. Again, this avoid having to support your weight and still allows you to move.
