v Design and construct individual dental appliances, such as retainers, with reference to the client's dental history.
v Regularly check and adjust dental appliances, this may include the braces getting damaged or they make break so making sure that they are fixed as soon as possible.
v Coordinate orthodontic services with other related services to ensure they are well-informed of the client's medical and dental history, so they need to make sure that they know the patients dental history from previous dentist.
v Always being honest with patients, especially when treatment is not required, or when options are available.
v Once the patient is in treatment, the orthodontist must try to do everything that is best for the patient. He or she should provide instruction on care and cleaning. They are responsible for finishing the patient's treatment with the best results possible.

What are the requirements needed to apply for a job?
it seems that you're trying to use negligence to sue your orthodontist. I'm no expert in law but based on the business law course that I've taken, i'd say you'd have to prove that your new orthodontist did not warn you of all risks/performed worse than average/deliberately w/held information to mislead you, etc. how poor a job your former orthodontist did has no bearing here.
There are no special requirements for one to fill out a job application, however, depending on the job one is applying for there may be specific requirements needed.
