No one will force you to give them your name and address. The telephone counselors answering calls are trained crisis workers. They will explore your feelings and help you come up with ways to cope with emotional pain. They may be able to give you referrals for local mental health clinics. In the case when there's an acute danger of you committing suicide, there's an option to send help to get you to the emergency room. It depends on your situation and each crisis center's policies.
You don't have to be feeling suicidal to call 1-800-suicide. If you're worried about someone else who is feeling suicidal or needs crisis intervention, they will also talk to you.
Answer from actual experience:
If you call and hang up, or they just feel like you're in danger of killing your self or someone else they'll 'ping' 911.
A ping too 911 gives them basic info such as 'you overdosed on tylenol'. First the police (and back up) will try knocking on your door, if no answer they break in.
If you answer the door, they'll ask your name and for ID. If you identify as the caller they'll tell you to turn around and put your hands behind your back as 'precautions' Congrats you've just been under arrest for the Mental Health Act. Once you're in handcuffs the paramedics talk to you a bit as-well as the police then they'll bring you to a hospital.

You don't get to wait in the normal emergency waiting room you get to wait in a psych room. Which is a room with nothing in it, reinforced door, high ceilings, and a camera. No one is allowed to talk to you except the doctors. The doctors will come and asses if you need to be seen by the psychiatrist on call. The resident psychiatrist will head down to you and talk to you if she feels like you need to be in psych ward then you're screwed.
