It's a square relay located in the relay center in the drivers side foot area. Alldata is not very clear but it looks like it will be on the inside of the door frame, or directly to the left of your left foot. It is also the fuse box. The relay you want is the second one down on the right. Or farthest from the electrical connections to the relay center, second one in from the corner.
According to my local chevy dealer, it is the emergency flasher relay button and flasher assembly all combined. $75.24.
