Does it work when A/C or defroster is on? What is engine temperature? with a thermometer Electric fans and clutch fans only operate when necessary, not full time, ie: engine is hot or a/c is on. Check the fan operation when the engine is in a condition where the fan is actually required. The 30 second run when the cool engine is started is essentially just the control module checking that the fan works.
Check the thermal relay for the cooling fans.
Bad cooling fan or thermal relay. Bad water pump, clogged radiator.
Blown fuse, defective thermal relay, or defective fan motor.
Is the cooling fan coming on as it should? If not replace the thermal sensor for the fan. Did you bleed all the air out of the cooling system after these repairs? You may very well have air trapped in the system. Us the bleeder valve if equipped. If not remove the radiator cap, set the heater to hot, and start the engine. Keep the cooling system full until you see no more air bubbles escaping.
You stop driving it and repair the problem that is causing it to run hot. A complete cooling system service is in order. Drain the coolant, replace the thermostat, replace the coolant, check the function of the water pump, the thermal switch for the cooling fans and the cooling fans themselves. Clean the exterior of the radiator and flush the radiator.

The cooling fan should run continuously when the A/C is on. See if it is running, and if not check the fuse, and thermal relay. If they are good, see if the fan motor itself it good. If all this checks out, the drain and flush the radiator with a chemical flush. Also make sure the front of the radiator is free of debris.

If you have a "Leak" from the cap either the cap needs to be replaced or the top of the Radiator is damaged. It is normal for thermal expansion (water gets bigger when hot) to cause the coolant to go into the expansion tank when cars is hot and to be drawn back into the radiator as the engine cools
