If you have manual transmission, most likely the clutch has to be replaced (as you said).
Not likely, it probably just revs the engine a bit higher before switching gears. Your MPG will suffer, but it likely won't damage your engine. If it did, they wouldn't include it in the car.

Starts you off in second gear. Also changes the ECM profiles giving you better revs.
S means "Sport"in automatic transmission cars, so that engine can be run in higher revs and more power can be used.
Remove the instrument cluster, (speedo, revs etc). On the back of it, there is usually two holes, one with a screw and one with out (sometimes it will say, "T-belt" near it. All you do is take the screw out and put it in the other hole. This works for most Hiaces
sports mode, simply holds revs longer before changing to a higher gear.
