There probably could be several causes. The two most commonly occurring of which I can recall are a defective heate core [a small radiator inside the heat and A/C plenum box] which allows engine/radiator coolant to leak into the plenum, and the second would be some foreign object [like a candy wrapper or piece of a plastic bag] being sucked into the plenum through the return grate, and then being caught on the blower speed control resistor[s] pack.
