They shouldn't be allowed to use your name without your permission. I would make it clear to them that they need to remove it immediately or you will seek legal recourse. If you have already tried this tactic and gotten nowhere, get an attorney and file a judgment against your previous employer. Don't worry, these things usually settle out of court, but it puts a little of the fear of God in people to think they will be sued and maybe that will be enough for him to remove your name. I've used that tactic successfully on a couple of occasions and it works!
