Not without going to court. If he has run the title so the car is in his name, the only claim you have is the prommissory note. Depending on how this is written, you may have few options. If it is open ended, just saying he owes you for the car with no specific date or payment schedule, he can always claim he intends to pay you at some point. A judge can make him do it in a reasonable manner or return the car.
Title for what? If it's for a car, and she has legal means to do so (power of attorney), she would sign the back under Seller and note P.A. after her signature.

Mow the neighbor's lawn ... knock on the door, ask $5 for a small yard, $10 for a medium, $20 for front and back etc. You could clear the snow off the neighbor's driveway. Take the neighbor's grocery list and do the supermarket shopping for her. Deliver a pizza or a sandwich for a small neighborhood take-out restaurant. Deliver a document or a package to a local address for a small neighborhood business. Walk the neighbor's dog or cat, and feed it, while the neighbor is at work. Take the neighbor's car in to get it washed or have the oil changed. Wash the neighbor's car yourself. Wash the outside of the windows in the neighbor's house. Note: You have to set it up with the neighbor, get their approval and agree on the price, before you start on any of these. Just knock on the door and offer to do it.
Real property can only be encumbered by a mortgage and not by a promissory note. A promissory note has no effect on real property it is only evidence of a loan. If the mortgagee acquires title to the mortgaged property the title merges and the mortgage is extinguished.

The best thing to do would be to talk to the neighbor first, if they are still mad at you send them a nice present. Whenever I do this they usually forgive me!
