2 wheel or 4 wheel drive, front or rear rotors, it makes a big difference.

There's 3 on that truck. You have 1 in behind both front brake rotors and 1 on the transmission tail shaft.
There is a tension adjustment on the driver side just forward of the rear wheel.
Brake rotor/rotors are out of round / warped. Check rotors and REPLACE as needed.
Yes. Although it confuses me as to why you would want to use old rotors for a brake job. New ones are cheap.

If you're changing the brake pads and there is too little rotor thikness to turn it, it should be replaced. If the rotor is warped and is too thin to be able to turn to remove the warping, replace it. If the brakes "pulsate" when you use moderate brake pressure, the rotor is warped.
Volvo pads and rotors are the best ones to use. Most aftermarket pads and rotors seem to make braking noise
remove wheel. remove brake caliper assembly(not the brake hose) and pull off the rotor.
Most likely needs the front rotors turned or replaced.
