Front deltoid tear (Shoulder muscle) and chest muscle tear. Actually, i would say that most bench press injuries are joint related, such as a rotator cuff tear or labrum tear. Bone spurs can form in the shoulder joint and start carving up the tissue around it if the shoulder is not controlled and secure when bench pressing, so be careful.
It depends on how the weight is lifted, but the most common injuries are back, leg, arm, neck and shoulder injuries (muscles, ligaments); blisters on the hands; joint pain in the ankles, knees, wrists, elbows and shoulder. Increased damage can also be done to the cervical and lumbar spine in the form of stressed or herniated disks, which will lead to further problems as the person gets older. If the person already has health issues, it can already compound those problems.
For example, I have a condition known as Spondylolisthesis, which is a hereditary condition that makes a person more susceptible to disk herniations in later life if certain activities are engaged in earlier in a person's life. Not knowing what the activities were many years ago, I was an avid competition weight lifter and football player in school. I also handled and lifted heavy weight during my Naval Submarine career.
Those activities have cost me later in life. As a result of my early life activities, my disks were stressed and finally herniated. They eventually got worse, and led to 3 separate spinal fusions and many years of opiate therapy. The end stage of Spondylolisthesis is when a disk collapses to the point where the vertebrae above the herniated disk slips forward and contacts the vertebrae below the herniated disk, resulting in bone on bone contact. It is extremely painful to say the least.
