Machining rotors allow a complete level surface for the new brake pads to contact, otherwise you would experience a spongy brake pedal when applied, plus braking capacity would be greatly reduced. It is not a gimmick to get more money. Call Midas, Discount Tire Center, or America's Tire to compare what they do and price for front brakes (if it is the front you are talking about).

Yes. It does not make a difference whether is is the conventional, slotted or drilled rotors. They can all be machined in a brake lathe with the same accurate result.

If your rotors are worn you should replace those as well. Worn, warped, or glazed rotors will reduce the effectiveness of your brake pads and possible wear them down faster. As far as the possibility of doing so, sure but it's not recommended. Just to add a comment, depending on the rotor thickness, the rotor surface should be machined to remove the glaze or warpage before installing new pads. Machining of a rotor surface reduces the thickness. If a rotor surface has previously been machined the rotor may need to be replaced.
