No it does not mean you are having a miscarriage, most likely you have hemorrhoids or anal fishers which are very common during pregnancy. You should tell your doctor so that he can prescribe you some hydro cortisone acetate or something similar to help sooth and heal the hemorrhoids. I am 29 weeks pregnant and have had this problem from the first trimester and it has helped me tremendously.
