I live in Israel, and in my experience a plus symbol hasn't been necessary (my phone doesn't even have one). Nor have I used the "00" in recent years.
To address the Question, I dial an international service (012, or 013, or 014, etc.), then the 1 for the U.S., and then the 10-digit number as any call to a different area code. So, for example, I dial 012-1-(aaa)-xxx-xxxx (with aaa being the area code).
