You can maintain your blood glucose levels by watching what you eat and exercising. If you are diabetic, you should get a blood glucose meter and test your blood twice a day to make sure you are within normal levels. Also, if you are diabetic, take your insulin.

Great question. Chronically elevated blood sugar is caused from having so much sugar in your system that despite high levels of insulin, the sugar spills back into the blood. To reduce blood sugar you need to reduce the amount of sugar in the body. There are three things you can do to accomplish this. First eliminate adding more sugar to the system. This means to stop consuming sugar (in any form) and anything that converts to sugar (starches like bread, pasta, potato etc) this is done by adopting a very low carbohydrate diet such as a Ketogenic diet. Next we need to get the body to burn off the excess sugar in the system, this can be accomplished with a combination or exercise (high intensity) and intermittent or extended fasts. Of course if you are on any Diabetes medicine, you must consult your physician prior to starting any of these measures. As they reduce you systems sugars the medicine can cause your blood sugars to get too low. If you are aggressively eliminating sugar from your body, you will need to taper and eventually stop your meds and this needs to be under the prescribing physicians care. You might enjoy ‘The Diabetes Code’ by Jason Fung as he is a physician, runs a large clinic dealing with T2D and speaks in easily understood language and makes good recommendations. Personally, I was formerly a T2D with an Ha1c of 9.6 and was taking 3 meds but not insulin; now I am one no meds, have lost a bunch of weight and my last Ha1c was 5.3. Good luck!
