Assuming this is a 4.3L engine, the upper intake manifold gasket would be between the upper intake manifold and the lower intake manifold.
The only fix for a bad intake gasket is to remove the intake manifold and replace the gasket.
by the lower intake manifold while you are doing this you should replace the intake manifold gasket anyway because it will leak sooner or later.
Check the lower intake gasket, or the thermostat...

Probably just a leaking intake gasket. When it's surging, try pressing downward (with most of your bodyweight) on the intake. If it settles out you need to replace the intake gasket.
Low Coolant. Poor coolant circulation. Intake gasket. Head gasket.

have four ports blocked on my 89 gmc 4.3 from the factory had since new,the four from the firewall were blocked with metal squares under the gasket
Upper intake gasket? Not much. It's a quick and easy repair (about 30 minutes) with a part going for $12 to $30. Figure on $45 to $160.Lower could be from $750 to $2600 depending on local labor rates and thoroughness of the mechanic. Aren't you glad yours isn't leaking?
