If they are not underwater you use an angle grinder to cut them flush with the concrete, then use a hammer and cold chisel to beat the metal in the concrete down below the surface. then make up a strong mix of sand cement mix it with a bit of bond-crete this will waterproof it and make it stick inside the hole make the top flush.
