To remove the rear brake drum of a 1993 Dodge Spirit I did the following:
Remove the protective cup in the center of drum, remove cotter pin and loosen the nut (DON'T REMOVE IT, just loosen it)
Raise rear tires from floor
Remove tires
Remove the nut in the center of drum
After that, you should be able to remove the drum. If it is stuck (like in vehicles that haven't been moved for long periods) you can carefully hit it with a hammer in its outer edge to loosen the brake shoes from the drum. Makes hits opposite to each other.
Assembly procedure is in reverse order of removal.
Tighten the center nut to 20 - 27 ft-lbs (27 - 34 N-m) of torque while rotating the wheel. Stop the wheel, loosen the nut and finger tighten it back. Install a new cotter pin.

first you have to take the wheels off of the van,and then you have to take the tension off of the brake shoes. Next you will be able to remove the rear brake drums off of the vehicle.You may have to tap them with a hammer to break them free.
Brake drums are not attached with fasteners, they just pull off once wheel is out of the way.
