there are small plastic tabs on both sides (driver's and passenger's) that lock in just beneath the top of the holder. the easiest way i found to get it out was to open the center console storage, remove the storage bin (it lifts right out), then reach in where the CD holder is and squeeze the sides while pushing upward. If you push one of the sides in far enough, the tabs will clear and then you can lift it out.

If I remember correctly, you really can't, it's part of the console so the whole console has to be changed.
To get to the ashtray bulb on a 1997 Z71, the console top needs to be removed. First remove the ashtray from its holder. Then, remove the clips on the sides holding the console top on.
The previous answer was a bit of a dick answer. There are three screws. You have to pull out the front cup holder and remove that screw (might take some work as mine had some glue on the end of it). Then remove the rubber bottom of the back cup holder and remove those two screws (torx screws, so you have to have the right tool). After that, you can pull the console out.
Remove (or have a professional remove) the center console. The cup holder may or may not be repairable.
There are screws going through the bottom into the floor, should be about 6 in the front, middle, and back of the inside of the console.
Remove the screws that fasten the console to the floor, there are 2 in the rear cupholder, 1 in the front cup holder, and 2 near the shifter (where the console seperates). When all the screws are removed you can lift out the console (be careful to unplug speaker wires), turn over, and remove the speaker.
