The rear bumper cover is held onto the frame by 4 bolts. They have a 13 mm head, and are accessed from under the van. They thread directly into the back of the bumper cover, therefore no nuts to worry about. The rounded edges of the bumper cover slide over a pin, so when the 4 bolts are removed, the bumper cover can be pulled straight off.

Look under the bumper cover itself and remove any pushpins. When all these fasteners are removed, the bumper cover should just slide off. If you want to replace the whole bumper (metal and styrofoam included), take it to a pro or grab a service manual.
