The Bernina Express does not run from Milan. It runs from Chur or St. Moritz, both in Switzerland, to Tirano in Italy.
Tirano to St. Moritz takes 2½ hours.
Tirano to Chur takes just over 4 hours.
You have to first take a train from Milan, or anywhere else in Italy, to Tirano.
From Chur you can then take another train to anywhere in Switzerland.

This depends where in Switzerland to where in Italy. Switzerland and Italy border each other, so the answer could be no time at all, if you are just crossing the border. Zürich to Milan takes approximately 3 hours 45 minutes.
