The amount of grains might depend on your age, sex, and level of physical activity. Most Americans consume enough grains, but few are whole grains. At least ½ should be whole grains.

Examples from USDA link below:

Women 19-50 years old = 6

Men 19-30 years old = 8

Men 31-50 years old = 7

In general, 1 slice of bread, 1 cup of ready-to-eat cereal, or ½ cup of cooked rice, cooked pasta, or cooked cereal can be considered as 1 ounce equivalent from the grains group. See USDA Link for more examples.

Ideally, none. The refining process removes the bran and the germ from the grain, and with them, much of the fiber, vitamins, and other nutrients. What is left, the endosperm, is mostly carbohydrate that is easily digested into sugar and stored as fat. It's much better to eat whole grains, such as whole wheat, brown rice, barley, and fresh corn.
