Take two forms of I.D. (Social Security Card and Birth Certificate), money for the replacement fee, and your guardian to the local DMV office. They will be able to make a new copy.
Go to the DMV and ask for a copy. It might not cost anything, but if you are stopped by the police, you cannot proceed. You need it with you at all times on the road.

It depends. Was there a licensed person in the car? If there was licensed person in the car with you, which would've made it legal for you to drive, then you will be cited for the open container and the DMV may impose an additional sanction, however, I don't think that you will lose your permit.
