The standard procedure for lumbar epidurals is 3 injections over a 3 week period. However, if the patient hasn't shown any improvement or relief after the second week, surgical options are considered. As long as the patient is improving, a doctor can make the decision to push the option a bit further.

If your doctor is proceeding with another injection, you must be responding. In any case, painful though they may be, epidurals are nothing compared to spinal surgery, and you should avoid any surgical option as long as other therapies are working.

Epidurals and facet injections are usually limited to less than 10 per year because of the effect the steroid used can have on your system. Most herniation's that require an epidural are scheduled with 3 in mind over a 3 week period, one for each week. However, in reality, if the patient isn't seeing any improvement after 2 weeks, they usually skip the 3rd and start talking about surgical options. If you're having enough spinal problems that require enough that's pushing 10, then other options should probably be considered.

Facet injections are arguably about as painful (NEVER look at the epidural needle), but they're more effective as they're targeted to the specific area of the herniation. An epidural is a "general area" injection. While the pain for either type of injection sucks, it's still not as bad as the disk pressing on the nerve.

It's important to note that there's not a hard and fast rule (like anything else in medicine). If your doctor feels the benefits outweigh the risk, they'll promote the idea, but in the end the decision should be up to the patient.
