Sounds like its getting hot and the thermal protection is shutting it down. Try cleaning it or oiling it.

Check to see if you are getting power to blower motor - if so replace blower motor If not check fuse, switch, resistor pack.

It is screwed to the bottom of the heater plenum - extending into the plenum. It is about 3 inches to the rear and about 2 inches to the left of the blower motor. It has a plug with 4 wires plugged from the bottom. Hope this is a suitable explanation.
