On the front side of the throttle body on the left side is a air valve with 2 tork screws. Remove it and clean the carbon out with gumout spray or replace it. That is the fast idle valve. that is why when you start your engine it revs and idles down.

My 4runner with 227000 miles on a 4 cylinder idles well at start up run it and it revs high at 2000 rpms between shifts and idling why?
Try changing your pcv valve. if that dont work it could be your mass air flow senscer or throtle pasistion senscer
