The fall of the western Roman Empire (the eastern Roman Empire was to exist for another thousand years) was not caused by any big military defeats. The Empire more or less imploded on itself because of a combination of factors.

On the one hand, central authority had weakened dramatically over the last century of its existence because of a never-ending series of violent revolts whenever another general proclaimed himself Emperor. Most emperors lasted only a few years and during their tenure they only focused on efforts to consolidate their power instead of on restoring the empire. Nevertheless, almost all were killed in the end by the people around them. The revolting generals usually took their troops along to Rome (if they made it even there), robbing the border regions of troops to guard against incursions, and the troops and local officials that remained did in the end not even get paid anymore by Rome.

On the other hand, the badly-guarded borders, sparsely manned by "Roman" troop remnants that often consisted of auxiliaries that were members of the same tribes that wanted to get in, were an open invitation to tribes that had been displaced elsewhere, outside of the Empire and now took a close look at the fleshpots of a prosperous and well-developed Roman Empire. Several tribes gradually established themselves in the Empire and even in Italy itself; and the last Roman Emperors already were often overshadowed by 'barbarian' generals and high officials. By then, large parts of the western Roman Empire had already effectively slipped out of Rome's control - stopping to pay taxes in the process, which in turn caused Rome's financial means and authority to deteriorate even further and faster.

In the end, one of the barbarian leaders simply deposed the Roman Emperor and took over - as King of Italy. By then, the other parts of the Roman Empire had already started a new and independent existence.

Why did Assyria fall to Persian and Medes?

The premise of this question, namely that Assyria was defeated by the Persians and/or Medes is incorrect. The Assyrian Empire was only ever defeated by the Babylonian Empire. These defeats were brought about by internal strife within the Assyrian Kingdom. The Persians were able to conquer Babylon and acquire the previously conquered Assyria.
