The fact that the person must initiate an effort first can initially be a drawback. For example, an overweight male is hospitalized after an episode of chest pain and shortness of breath. The diagnostics determine that the pain was not cardiac but likely from a hiatal hernia related to obesity. The physician tells the man that losing weight will help both the hernia symptoms and help improve respiratory function. But to many overweight patients, to exercise and lose weight seems like an overwhelming task. However, if the patient can begin the exercise regime, he will begin to see some results within 2-6 weeks. In the meantime, the positive reinforcement others give must focus on intangible things like the patient's determination, will, and self-pride. Once weight loss begins to occur, the behavior should begin to be self-sustaining because the results become a big part of the reinforcement.
