1. Classical Music comes in eras for example, baroque, renaissance, romantic etc.

2. Probably the most famous composer would have been Mozart.

5. It started fading away in its birthplace, northern Italy, around 1600, being replaced by the Baroque era.

Beginnings of Baroque music

Italian composers such as Monteverdi had to create another new musical period in order to write operas, which incorporated things like basso continuo. Once the baroque style was invented, Monteverdi and others would not abandon it in future compositions.
