My dog had this,it's called an earflap hematoma,it happens when a dog shakes his head and hits his ears on something,I saw my dog do this on an end table,and blood vessels burst filling the ear-flap with blood.They may do it scratching their ears and may be a sign of infected ears but not was the case in my dog.I tried to get him in the Vet. and they were all booked for at least two weeks.And I knew the longer the fluid remains in the ear the thicker it will become.So,I cleaned his ear and took a syringe,I purchased at a feed store,and drew the fluid out. It worked well and with little pain and we're both happy.
