It depends on the religion.

For Christianity, it's Christian.

For Islam, it's Muslim.

For Buddhism, it's Buddhist.

For Wicca, it's Wiccan.

For Vodou, it's Vodouisant.

You can also call someone an adherent - eg, "she is an adherent of Buddhism."
