There is no "standardized" form for police reports and they can vary greatly according to the requirements of the reporting agency and the laws of the jurisdictions concerned. For example, in a basic Criminal Offense Report usually all that is required is that the following 5 questions be answered, if possible, (Who -What - When - Where - How) and a short narrative report describing the incident or offense. There are MANY MANY types of police reports and just as many forms on which to report the information (traffic accidents - criminal investigations - warrant applications - etc - etc. so it is impossible to definitively answer the question.

In Canada:

First page: name, address and DOB of the accused. There is a short description of the offence, and the relevant sections of the criminal code are mentioned.

Most pages are signed by the police officer, with badge number

Next pages are usually written statement by victims and witnesses. Its handwritten, and signed by police and witness.

You also find in this section a summary of the steps taken by police during the events (eg, detention of accused, interview with witnesses, contact with lawyers, ...)

You can find at the end a list of accused and witnesses, plus a list of evidence seized.

For a case not too complex, a police report is about 10-15 pages.

Looks like two or three pieces of 8 and a half by 11 inch paper. Victim information and bike description and serial number (if any) should be listed at about the middle of the page.
