The word "excision" means to cut out or remove. In this case, it probably refers to making an incision (a cut) into the area of the collarbone and removing a piece of tissue for biopsy. It could also mean removing the entire collarbone, or "clavicle" as is it's proper name. The clavicle is made of two parts, basically. You have one on each side of the upper part of your chest that form part of the group of bones and muscles supporting your shoulders. Removing a piece of the collarbone for a biopsy would mean that the piece removed will be sent to a laboratory for examination for any problems with the tissue. Removing an entire clavicle, either left or right, may occasionally be done because of problems with the bone itself.
