Depends on the mileage on the vehicle. This is an interference engine and as such, if the cam belt breaks there will be serious engine damage. On the 4 cylinder replace the cam belt, balance shaft belt, & water pump, which is driven by the cam belt at 105,000 miles. On the V6 replace the cam belt & water pump at 105,000 miles. At 100,000 miles replace the spark plugs. Replace air & cabin filter every 30,000 miles. Have automatic Transmission Fluid changed every 75,000 miles. Drain, flush, and replace coolant at 75,000 miles. It may or may not have an external fuel filter. If it does change every 60,000 miles. Rotate & balance tires every 10,000 miles. Change brake fluid at 100,000 miles. Inspect brake pads and change as necessary.

Honda recommends changing it at 100,000 miles. I personally recommend you change it every 75,000 miles. The 2003 Honda Accord had problems with the transmission so I would recommend this as a preventative measure.
