I would bet the fuel sending unit is the culprit. Go to Walmart and buy 2 bottles of Techron in the auto parts section and add them to your gas tank. Not both at the same time.......use one and then when you fill up again add the other. GM's fuel pumps and sending units are ntoroious for fouling up with sulpher and Techron removes the sulfur from the fuel sending unit. I own a 2004 Corvette and this same problem has plagued many of the Vette owners. It may take up to 3 tank fulls but this usually eliminates the problem. I would try this before I went looking anywhere else . And when you do buy gas, try to buy what is called top tier gasoline. It is not the most expensive, but it has the additives already in it to dissolve this sulpher.
Do a Google search and type in "top tier gasolines" and read what gasolines have these cleaning agents. I hope this fixes your problem.........I figured it would be a good place to start and you have nothing to lose other than the cost of the Techron.

A cars tach jumps and runs with engine rpm.Sometimes if it jumps and changes engine sound,the throttle position sensor needs changing.If it jumps with no engine speed or sound change it means your alternator is going bad.
Check for excessive slack in the timing chain.
I have 91 chevy k1500, It does this, Sometimes it will idle a few minutes, then jump to high idle and check engine light comes on, And sometimes its as soon as i start the engine... I was told it was a vacum leak, But im no mechanic
You will definitely know if your truck jumps out of time. The engine will run very rough. The engine might not run at all.

The timing chain has no regularly scheduled time or mileage for replacement. It will normally last the life of the engine. They are routinely replaced when a complete overhaul is done. You will know if it needs service. It will become noisy, and your engine may not run correctly if it jumps a tooth.
