Idle is set automatically by the PCM utilizing sensor on th intake manifold to limit the Idle Air Control Valve. If any of these sensors malfuntions or unfiltered air is entering the manifold, your idle will be up and down continously when in park or neutral.
Honda Accord 1998-2002 model GEN 6 idle speed is control by internal computer. You need to bring it to the dealer to adjust this.

A 1998 Honda Civic shakes when it idles usually due to worn engine mounts. Open the hood and check the engine mounts. See if they are either cracked or brittle. Since the mounts can not absorb the vibrations it will be felt in the cabin.
