The head light module is located under the center console near the front. Pull out the top pieces and should be in the right front corner inside the console.
It is in a plastic box and can be pulled out with out much effort. The soldered connections on the board work their way loose. If you can manage a soldering iron you can flow the solder back and reuse the module. Hope this helps.
The main headlight relay is mounded in the fuse box under the air filter. The relay for timed headlights is in the relay box under the center of the dash.

I was looking at the 1996 Ford Explorer Owner Guide and it shows : An arrow pointing to the passenger side of the engine compartment , for Engine compartment relay module # 2 ( it contains 2 large relays and 2 small relays and the front washer pump relay is the small relay at the bottom ) * I believe the relay box is actually between the passenger side headlight and the engine air filter housing , but I'm not a mechanic / technician , I just noticed there is what looks like a relay box there on my 1995 Ford Explorer
