Firstly, no warning system.That, and the fact there is a large population living on the coast.Not forgetting the tremendously destructive power of a tsunami and lack of education of such an event.The latter not just confined to Sri Lanka.

Unlike in the Pacific Ocean there was not tsunami warning system in the Indian ocean at the time. So most places were stuck without warning. A lack of education also contributed. Many did not realize that the ocean receding was a sign of an incoming tsunami.

Tragically during the 2011 earthquake, the tsunami that struck was bigger (up to 110m) than the walls (15m tall max) had been designed for. Furthermore, the tsunami warning system had a mistake in the analysis of the tsunami, predicting a much smaller (3m - 6m) wave height than actual. In addition The 9.0 magnitude earthquake was the largest in Japan's recorded history and many people could not receive the warnings due to power outage or other damages. A survey by the Japanese Government found that 50 percent of those within the tsunami's range received no warning at all. 42 percent who received a warning did not evacuate. 49 percent of those who did not evacuate were "hit by water." Most of 20,000 deaths was by drowning.
