It probably needs a tune up. My 1998 Mazda 626 was idling very rough, so I did a tune-up by replacing the spark plugs and wires. This made the engine run better, but the rought idling continued. I took it to my mechanic, and he found a vacuum leak somewhere, and $270 later, it idles smooth as glass
It is more than likely a thermostat
It is possible that your radiator needs to be topped up or that it is empty. My little 1991 Mazda Autozam Revue did the same thing so i checked the radiator and both tanks were just about empty.
If your air conditioner is in the on position it will change the sound of the engine each time it cycles on and off (pulling more energy from the motor) to keep the cab at the temperature setting you have it on.

if it's idling rough, I would suggest cleaning the throttle body with carburetor cleaner.
Could be your Idle air control valve.
The spark plug gap can cause the RPMs to fluctuate, while idling. The more probable cause of the fluctuating RPMs is a bad camshaft.
