either your clutch is not disengaging completely or your synchro's are going or have gone bad.
If you have a 4 cylinder model I'll bet it's the synchros, those ax5 transmissions are famously weak and suffer from synchro problems.

You may have to have your clutch adjusted,
Your transmission synchros might be going out. Also, shift slower and see if that helps. A Jeep isn't a racecar, so shifting slower is easier on the transmission. Changing transmission fluid to a different brand might also help your problem.
Heat shield on your exhaust system?
Your synchro's are going bad, common problem with the ax5. The synchro gear will match the speed of the two gears so they meet smoothly, when these wear out you will get difficult shifting and some grinding.

the plate that holds the shifter to the trans is loose, you should be having more shifting issues as well. Remove the plate that goes around the shifters on the floor to access the mounting bolts and tighten them.
