All modern First Ladies visit different countries, as part of their duties. Where years ago, First Ladies either stayed in the background or stayed in the USA, today, it is not unusual for a First Lady to travel internationally, and not just on vacation. For example, Mrs. Obama represented the United States at the 2012 Olympics in England, where she met with the Queen and the wife of the British Prime Minister in addition to leading the US athletic delegation.
As for why Mrs. Obama has not yet visited Arab countries, that is probably because the American Secretary of State, Hillary Clinton has frequently visited those countries, making it unnecessary for the First Lady to do so. Barbara Bush did in fact travel with her husband President George H. W. Bush to Saudi Arabia, but it should be noted she went there to visit and support American troops during the Gulf War. And there may also come a time when Mrs. Obama will have an occasion to visit an Arab country at some future date.
