I have an '03 EB with this issue and was told my fuel pump is going out. It can't build enough pressure quick enough to start the truck right away. I used to have to crank the heck out of it for 30 seconds before it started, but it was suggested I just turn the key from off to on (not start) about two or three times then crank it over and it starts just fine every time.
