During the beginning of his internship, he was picked by Dr. Preston Burke to perform the first surgery of all the interns. During the surgery, he choked and earned the nickname "007" (James Bond's License to kill)
I just researched this after watching the season finale! Something like it was his nickname given to him 1st season, something about having a "License to Kill" after losing a patient. They make a big deal of it in an episode because George hates being referred to as that. I'm a pretty avid fan, and I didn't remember that. I also missed the first hour of the finale tonight though, so I'm wondering if "007" was referred to at all during the first half. Anyway, OMG, what an outstanding ending!!!
It's from the first episode. George is doing a surgery with Burke and he freezes because he doesn't know what to do. The patients bp drops really low, so Burke pushes him out of the way and finishes it himself. Alex says he's a 007, Izzie( i think) asks why he's calling him that, Meredith says it means "License to Kill"..then George finds out everyone's calling him that.

Burke enters, tell O'Malley to show him what he can do. George asks for the scalpel, and all the interns in the gallery cheer, Burke makes a motion for them to shut up. (From gallery)Cristina says that Burke is trouble, Izzie laughs. (Back in OR) Burke glares at them. George starts to cut, but doesn't put enough pressure on it, Burke tells him more pressure that the human skin is tough shell, he should dig in. George starts the surgery, doing everything okay so far. (From Gallery) Cristina says Damn he got it. Meredith says told you he's gonna pull it off. (Back in OR) Again Burke glares. George takes the appendix out and you hear cheers from the gallery. Burke tells him that it's not bad, George thanks him. Burke says now all he has to do is invert the stump into the cecum and simultaneously pull up on the purse strings, but be careful not to…(George breaks them)break them. Burke says he ripped the cecum and he's got a bleeder filling with stool and asks George what he does now. George doesn't answer. Burke tells him to think. Nurse says that the BP is dropping. (From gallery) Cristina says he's choking. Meredith is still cheering him on. (Back in OR) Burke tells George to pull his hand out of his back pocket. George freezes. Burke asks him what he's waiting for, and tells him suction. Her blood pressure drops really low. Burke pushes George out of the way and calls him an idiot. (From gallery) Alex says he's a 007. Other interns agree. Izzie asks what a 007 is. Meredith says it's licensed to kill.

George thinks everyone is calling him 007, he asks Izzie and Meredith if they are, they deny. Cristina asks how many times they have to go through this? And she asks George to give he ra number or else she's gonna hit him. George says that Murphy whispered 007 on the elevator and everyone laughed. Izzie tells him that he wasn't talking about him, George asks if she's sure. Meredith asks him if they would lie to him. He says yes. Cristina states 007 is a state of mind. George says "Says the woman who finished first in her class at Stanford."
Yes in the first hour of season 5 finale once they found out George is leaving to go to Iraq to be a Trauma Paramedic, they all started going on about that 'George could not join, he is 007, he will kill himself from cleaning his own gun' and other stuff along those lines. He got this from the first series where he 'choked' on a major surgery for the former interns.
In the very first episode of Grey's anatomy George was performing an appendectomy and nearly let the patient die. 007 as in James Bond Licence To Kill.

George O'Malley ran in front of a bus to save a woman. The woman survived, but George was hurt so badly that even his own friends at Seattle Grace Hospital couldn't identify him. Eventually, Dr. Callie Torres (George's ex-wife), was able to identify him by a birthmark on his hand. He died in the hospital.

When Meredith realizes that it is George they take him into the ER but he dies on the table.
Just to make sure that it is George, Torrezz goes in and says that it is from a freckle on his palm.
Izzy also survives from her Cancer.

In the season 5 finale (part one) Izzy decided if she wanted the surgery that could result in memory loss but she chooses to have the surgery. George decided to join the army and Owen decides to go see his mother with Cristina for the first time since he got home from Iraq. In part two Izzy wakes up from her surgery but has some temporary memory loss Alex works hard to get her memory on track but gets frustrated. Meredith and Derek get married on a post-it and a man hit by a bus comes into the ER. Everyone finds out about George joining the army and decide to have an intervention to stop him after his surgery with the chief. The chief comes out of surgery without George and told Bailey and Callie that he went home. Izzy gets her memory back after Alex yells at her and Alex apologizes. Alex hugs Izzy and she codes in his arms. Meredith helps the victim of the bus accident he writes double o 7 on her hand (which was Georges nickname in season 1) Meredith realizes who it is. In the end both Izzy and George are coding no one knowing who will live
