There are spouses that even when death knocks at their door they are spouses forever. To send a card on an anniversary when the spouse is diseased isn't appropriate either and could well cause pain for the widow/widower. It's best to visit the person and let them do the talking about all the wonderful memories they had with their spouse and how much they miss them.
