Try to spray the moving parts of the fan with silicone spray. the fan only kicks on when the radiator is hot so remove the fan and wire it to a battery to get it to turn. That should help break apart any dirt and debris. It's probably time for a new fan soon after that. Nothing last for ever.

the blower resistor controls the fan speeds
Check the fuse and relay first. Then check the blower resister, then the blower motor itself. One of these is probably the cause of the failure.
check your fuse, check the wiring to the blower or the blower motor itself could be shot.
blower motor resistor needs to be replaced
Check your radiator fan switch and your fan switch coolant sensor .
Check the 30 amp Blower fuse. If not that check the fan relay, if not that check the blower motor directly from a jump on the fan itself.
Blown blower motor fuse? Bad resistor pack at blower motor? Bad fan switch?
the blower fan rheostat usually located in blower box is like a resistor that draws energy from the blower fan supply to slow it down and there is a high speed fan relay for the fastest speed. So your rheostat is not working
replace the blower motor resistor
