THE ANSWER TO THE PROBLEM IS AT THE BOTTOM... I, too, have the same problem. There is no problem with the brake light activator switch (as evidenced by the "3rd" brake light working), no fuse problems (I have checked fuses 40 & 45 as the manual references these two 10A fuses as being involved with the brake lights), the lights function properly in their other uses... Just not when braking. This happened to our 2005 Toyota Sienna LE at 37,000 miles and at 39 months. The repair diagnosis is a minimum of $300. So far, all Sienna owners I speak to (7 so far) like many of the features but all of us go through tires, fluids, problems with doors, windows and electrical. I will not buy another Toyota. My 1998 Subaru Legacy Outback is incredibly maintenance-free considering how sloppy it was built, this 2005 Toyota Sienna brake-light problem is costing me more than my recent 90,000 mile service visit. So far, Toyota plays ignorant... The best thing to do is to call Toyota and tell them you have seen others with this problem and hassle them. At least get your name on file as reporting it. They know full-well that the light is a DUAL-FILAMENT bulb. They do not say so in the manual, they do not announce it when asking questions. The single bulb acts as both stop and tail lights with each filament lighting at the appropriate command. If you are in a hurry, you can take the bulb out and put it in 180 degrees turned and it will probably work on when pressing on the brakes. All it takes is a 10mm socket to loosen the two bolts, there are two pins that press into the body and it is a very simple job. The bulbs originally installed were cheap mothers too. There's a wide gap between an ethical system and what I experienced. No more Toyota.
