It depends on the source. Do you get your water from the water department? Strike out there. Spring water from an under ground spring? Ding ding. Purified, city, drinking, distilled and even some bottled spring water has been filtered to remove most contaminants and minerals. Well water is very rich in minerals. It should be checked by a lab to make sure that there isn't to much mineral or harmful other contaminants

Bottled water, although seeming more pure and more luxurious than regular ol' tap water, is not that different from the tap water. When you look at the bottled water and look at the tag, it will say that the water is from a public source. If so, it's just filtered tap water. In fact, buying a filter for your sink is whole lot cheaper and maybe even cleaner than bottled water (bottled water has plastic because of the bottle).
It depends on what area of the world and what country you are in and from where you get your tap water. In the Chilterns in the UK it is built on limestone. The tap water is very hard. In the Lake District and parts of Scotland in the UK, it is much softer. It is the same in the USA.
Mineral water by definition does contain a high quantity of minerals and can be quite hard. Without analysis it is not a straight yes or no answer.

If you mean the condensation on the shiny surface of the bottle soon after you've filled it, then it's the cold surface cooling the air around it causing that part of the air to be able to hold less moisture vapour, and this vapour condenses out as liquid. Once the bottle has warmed to the natural air temperature this won't happen any more.

Soft or hard water has to do with the amount of dissolved minerals in water and what those minerals are. "Harder" water has more dissolved minerals (like calcium.)
